Short request letter (template)

Dear [name],
I am writing to request [support], as outlined in the attached evidence. This support is required because [one-sentence functional impact]. The evidence attached shows [brief summary]. Please confirm whether this can be included and advise next steps.

Correction request (template)
Script: "I request a correction to [document/record] dated [date]. The correct information is [one-line]. I have attached supporting documentation." Keep it factual, provide the evidence and ask for acknowledgment.
