What to ask clinicians to include

  • Clear functional assessment — how the condition affects daily tasks
  • Concrete examples of need (dates, tasks, severity)
  • Explicit recommendations: the support, frequency, duration, and expected outcome
  • Clinical reasoning and evidence (tests, observations)
  • Therapist credentials and signature
