  • Percentage of tasks with agreed written follow-up within 7 days
  • Participant satisfaction (short 3-question survey after each quarter)

Invoice & time checks

Ask for simple supporting notes: date, activity, time spent and outcome. Compare billed time with agreed time in the Service Agreement.

Activity — KPI checklist

Family KPI checklist:
• Monthly activity log received? (Y/N)
• Were follow-ups completed within agreed timeframes? (Y/N)
• Any unplanned tasks billed? (list)
• Participant rating of coordination (1–5)
