Types of coordination

Coordination can be general (helping link services), specialist (complex cases), or intensive (high need). Each has different expectations and time commitments.

Common coordinator tasks

  • Help identify and connect to services
  • Support planning and prioritisation of goals
  • Facilitate meetings and referrals
  • Monitor implementation and follow-up
  • Provide short-term capacity-building for families

What coordinators should not do

They should not provide long-term clinical therapy, replace case management for clinical needs, or make decisions on behalf of participants without consent.
